1. Recognition of risks

I acknowledge that access to thermal facilities (Nordic spa including alternating hot, cold and rest) involves risks:

• Physiological reactions linked to temperature variations,

• Adverse effects related to my particular health condition,

• Any other unforeseen effects arising from the use of the facilities.

I declare that I have read these risks and assume full responsibility*

2. Medical consultation

I acknowledge that it is my responsibility to consult a healthcare professional (physician, oncologist or other specialist) before using the facilities, to ensure that my medical condition allows me to access them safely.

By signing this document, I confirm that:

• I have consulted a health professional or

• I have chosen, under my own responsibility, not to do so and I fully assume the risks involved.

3. Disclaimer

With full knowledge of the facts, I fully release ÈST Éco-cabines , its owners, employees, representatives and partners from any liability in the event of injury, discomfort, medical complication or any other damage that may arise directly or indirectly from the use of the facilities.

I waive any recourse against ÈST Éco-cabines for any incident that may occur during or after my use of the thermal facilities.*

4. Declaration of accuracy of information and sanctions in the event of fraud

I certify that the information I provide regarding my health status and my eligibility for the program is true and complete .

I understand that any false declaration regarding my health status or my eligibility for the program constitutes fraud and that, if such fraud is discovered, ÈST Éco-cabines reserves the right to charge me the full cost of access to the facilities .*
